“Filaggrin”, it’s all in the Genes!

What is Filaggrin?

Filaggrin is a protein that is essential for skin hydration and the integrity of the skin barrier. It is largely responsible for how well your skin retains moisture and determines how hydrated and healthy your skin is. Filaggrin is essential for the production of Natural Moisturising Factor (NMF) – substances that maintain skin hydration and skin barrier function.

Filaggrin is resultant from the breakdown of Profilaggrin; a protein contained in the granules of keratinocytes within the granular layer of the upper epidermis.

People with eczema-prone skin may be deficient in the gene for making filaggrin, which may result in a shortage of this protein. A lack of filaggrin makes it easier for moisture to escape as your skin barrier is compromised, making your skin vulnerable and prone to dehydration.

Filaggrin is a major structural protein in the stratum corneum of the epidermis. Mutations in the filaggrin gene are the most significant known genetic risk factor for the development of atopic dermatitis. Mutations in the human filaggrin gene (FLG) also confer risk for the associated allergic diseases of food allergy, asthma, and allergic rhinitis. These discoveries have highlighted the importance of skin barrier function in the development of atopic diseases, motivating a surge in research characterizing the filaggrin-deficient skin barrier and its consequences.

 

 

Filaggrin function:

Compromised Filaggrin formation does not allow for the cutaneous cholesterol emboli (CCE) to form correctly, resulting in the corneocytes drying out due to a lack of adhesion of the multi-lamellar lipids. This, in turn, results in a poorly functioning immunological barrier (how your cells respond to stress) due to the loss of necessary lines of skin barrier defence.

A compromised skin barrier function also allows for easier entry of allergens, triggering inflammatory & allergic immune responses (such as atopic eczema and allergies).

It is imperative to keep in mind this deficiency can then be an aggravating factor or the leading cause in a variety of skin disorders & diseases and will be evident in family history.

How does filaggrin deficiency occur?

We are made up of thousands of different pairs of genes that are passed on from each of our parents: one set from each of them. Around 1 in 10 of the population have reduced amounts of filaggrin because they have inherited a faulty copy of the gene for making filaggrin during conception.

A person that has a filaggrin loss of function gene inherited from one parent will have around a 50% reduction in the amount of filaggrin produced in their skin. Many factors that need consideration: hard water, chemical history, clinical treatments, environment, stress, medication, diet, pediatric history, family history and so much more will contribute to the skin’s integrity that is already challenged due to the loss of filaggrin.

If you have inherited a filaggrin loss of function gene from both parents, you will have no filaggrin and most likely will have symptoms of Neurodermatitis, Ichthyosis Vulgaris, and Atopic Eczema persisting into adult life.

How can you tell if you are Flaggrin deficient?

Visual clues to determine if you are filaggrin deficient: Very dry skin (lipid dry) leading to loss of water and impaired enzyme activity, dry hands with increased linear creases on the palms over the base of the thumb, fissures (cracks) on hands and feet, Keratosis pilaris (a common, harmless skin condition that causes dry, rough patches and tiny bumps, usually on the upper arms, thighs, cheeks or buttocks. The bumps generally don’t hurt or itch.) also has an association with filaggrin deficiency, skin disorders such as psoriasis, eczema, dermatitis
